  • Personalized direct marketing
  • Send brochures, conduct physician office visits & telephone calls to target patients and potential referral sources.
  • The marketing goal is to receive 2-3 referrals per month, with a 90% conversion from referral to admission rate.

Eligibility for
Adult Foster Care Program

To participate in the Adult Foster Care (AFC) program, individuals must meet specific eligibility requirements. Participants should be at least 16 years of age and require ongoing care and supervision due to a medical diagnosis or the need for assistance with Activities of Daily Living (ADLs) such as:

  • Bathing, dressing, eating, and toileting
  • Transferring (getting in/out of bed or a chair)
  • Ambulation (walking and mobility support)
  • Managing behaviors such as wandering or disruptive actions

Additional requirements include:


This program ensures individuals receive the necessary support while remaining in a safe, comfortable home environment.

  • Health Insurance – Must have insurance that covers Adult Foster Care services
  • Qualified Caregiver – Cannot be a spouse, parent of a minor, or legally responsible relative
  • Approved Home Setting – Must reside in a qualified home setting within the state
  • Medical Approval – Requires a signed order for AFC services from a primary care physician
  • Recent Physical Exam – Must have undergone a physical examination within the past three months

Caregiver Eligibility and
Responsibilities

Qualified caregivers can be either family members or non-family members, except for a spouse or legal guardian, who are not eligible for this role.

Eligibility Criteria for Caregivers:

  • Must be at least 18 years old.
  • Can be a relative, friend, or a trusted individual from the community who is willing to live with the client and provide necessary care.
  • Must be committed to offering assistance, supervision, and hands-on support as required by the client’s personalized care plan.
  • Responsible for ensuring the client’s safety and overall well-being at all times.
  • Must consent to a criminal background check (CORD).
